Job Description



Summary

The STOP Case Manager (Program Counselor) is the direct provider of casework services to the Participants, including intake, initial assessment, and initial placement in community based services, transportation, case planning, case management and interfacing with DAPO (Parole).  This is a non-exempt, hourly paid position.



Primary Duties and Responsibilities

Statement of Responsibilities:  Responsibilities include, but are not limited to, the following:

a.             Acting as a role model, modeling professionalism and pro-social life skills and behavior patterns at all times consistent with the substance abuse treatment services being provided to the STOP participants;

b.            Provide interactive services to the STOP Participants.

c.             Develop Case Management Plans (CMPs) for Participants.

d.            Develop and monitor the Participant's progress relative to their CMP through all phases of the program.

e.            Make appropriate referrals to outside agencies as necessary.

f.             Maintain progress notes in Participant files.

g.             Keep the AOR apprised of Participant's progress.

h.            Develop discharge and aftercare plans.

 

Job Requirements



Minimum Requirements

Possess a high school diploma or its equivalent and a minimum of three (3) years experience working with criminal justice populations.

At the time of hire, must be certified as an AOD Counselor with a certification recognized by Department of Health Care Services (DHCS) http://www.adp.ca.gov/Licensing/LCBhome.shtml (Licensed professionals, including a licensed physician, psychologist, clinical social worker, and registered interns are exempt from this certification requirement.).
