This is a re-entry/release coordinator role
Summary
This position is responsible for providing re-entry services to the inmates.
Primary Duties and Responsibilities
Develops a list of inmates who are scheduled for discharge the following month.
Meets with inmates who are scheduled for discharge the following month to have the inmate fill out a Re-Entry Questionnaire.
Complete and process COMPASS applications for inmates requiring it prior to discharge.
Utilizes the filled out Re-Entry Questionnaires to provide the inmates scheduled for discharge with the following items:
•A General Re-Entry letter
•Delaware County Resource Guide
•Other resources requested by the inmate
Prepares a Discharge Summary for each inmate scheduled for discharge the following month that is currently on Parole/Probation or will be on Parole/Probation once released and sends the Discharge Summaries to Delaware County Adult Probation and Parole.
Reviews all inmates convicted of an offense mandating DNA collection by statute to determine if the inmate’s DNA is already on file with the State of Pennsylvania utilizing the Commonwealth Law Enforcement Assistance Network (CLEAN).
Collects DNA and fingerprints for all inmates convicted of offenses requiring DNA by statute, fills out all necessary paperwork, and submits DNA and fingerprints to the Pennsylvania State Police.
Handles all Megan’s Law Registration requirements for all inmates incarcerated here at the facility.
Creates and maintains excel spreadsheets detailing all reentry resources provided, DNAs researched/collected, and Megan’s Law registrations/updates.
Provide Transitional Case Management for a caseload of sentenced inmates preparing to be discharged
Performs other duties as assigned.
Minimum Requirements
High school diploma or equivalent certification required.
Bachelor's degree in criminal justice, sociology, or related field is preferred.
Must have valid Driver’s License with a clean driving record
One (1) year in caseload management of offenders in a correctional setting preferred or an equivalent combination of education and related work experience preferred.
Must be able to pass a background, NCIC criminal history, drug screen and medical evaluation.
