Job Description

This position will assist with the coordination of services for client care, maintaining all required documentation of case management.  This position is responsible for bringing services, agencies, resources and other people together to assist a client in achieving established goals.  This position will report to Case Manager Supervisor/Clinical Director.


Essential Duties and Responsibilities

  • Provide case management to coordinate other services, resources and people to assist clients in achieving their established service plan goals
  • Screen and assess clients for their appropriateness for agency services
  • Adheres to the service hour protocol established by the agency
  • Implementation of the therapeutic community treatment model under the direction and supervision of the site specific supervisor
  • Coordinate participant's linkage to community treatment services and support engagement in drug treatment, community, etc.
  • Provides comprehensive Re-Entry Education/Skills Training services as well as other Cognitiv Behavioral Positive Peer Community methods and services
  • Works with a team in the development of appropriate Positive Peer Community behavioral interventions for individual residents and or resident community
  • Coordinates individual clinical care of assigned client caseload related to case management
  • Participate in treatment planning/conferences
  • Facilitate individual and/or group orientation or educational sessions (as required)
  • Provides individual and group therapy/counseling. Meets with assigned clients as necessary focusing on after care and re-entry
  • Maintains and updates resident client's records; documents program in charts at a minimum of every 30 days per client
  • Completes reports/referrals for the purpose of Re-Entry Transitional Planning
  • Preparation and submission of all treatment and educational outlines, lectures, handouts, and other relevant materials as requested by the site specific supervisor for review and approval
  • Document the results of assessments, service plans, reports, progress notes, discharge summaries and other client-related activities including accurate reporting of services
  • Record keeping and daily clinical charting as required by the GEO Reentry Services program model and DASA licensing requirements, and/or as requested by IDOC
  • Attend in-service trainings, conferences and education sessions (as required by GEO Reentry Serives and IDOC)
  • Complete and maintain all required documentation pertaining to clients (documentation, letters and status reports)
  • If not certified, obtain a CADC or reciprocal credential within two years of employment

Job Requirements

Minimum Requirements

Bachelor's degree preferred in human services, social work, psychology or other related fields and one to three years' experience in human service activities; or any equivalent combination of training and experience that provides the following knowledge, abilities and skills.  If not certified, obtain a CADC or reciprocal credential within two (2) years of employment.
