The Case Manager II works as part of the treatment team, especially in treatment and discharge planning, with primary responsibility for helping clients gain access to a wide variety of supportive services. The case manager must also document all client contacts accurately and thoroughly, and must work to expand Abraxas’ linkage agreements and relationships with other service providers.

Under the supervision of the Treatment Supervisor the Case Manager II is responsible for the development of the program and supervision of other staff.  The Case Manager II coordinates treatment by providing chemical dependency counseling services to clients.

Primary Duties and Responsibilities

  • Effectively uses supervisory skills to select, evaluate, counsel and train staff.
  • Effectively uses written, verbal and non-verbal communications to convey ideas and supervision to subordinates, other employees and clients.
  • Provide daily case management to staff through supervision and staffing and to clients through assessments.
  • Provide clinical services through group and individual counseling. Participates in weekly meetings and staffing.
  • Recognizes and distinguishes between productive and non-productive conflict between staff, clients and outside influences and appropriately uses effective conflict resolution techniques to respond to problems, conflict and grievances.
  • Plans, structures and facilitates various recreational sessions while developing therapeutic relationship in addition to identifying possible barriers to treatment.
  • Creates a positive and productive work environment for staff through facilitating team relationships, organizing team goals, assessment of performance and feedback to team.
  • Identify potential conflicts and implements crisis intervention techniques to prevent or intervene in potential crisis situation.
  • Interacts with other agencies, professionals, referral sources, funding sources and contracting sources by presenting a positive image of him/herself, the facility and the organization.


Job Requirements

Minimum Requirements

  • Possession of a Bachelor's Degree from an accredited college or university with major course work in social work, psychology, counseling, sociology, or a related field and three years of social service experience, one year of which has been with substance abuse and/or mental health clients;or equivalent to a Master's Degree from an accredited college or university with major course work in social work, psychology, counseling, sociology, or a related field and one year of professional case management experience involving substance abuse and/or mental health clients.
  • Possession of a valid Illinois driver’s license and car required. Preferred licensure/certification includes Certification in Alcohol and Drug Counseling (CADC), licensure in clinical social work (LCSW), certification in counseling mentally ill substance abusers (MISA), or related.

Preference is given to the following credentials:

  • Master of Arts/Science Degree in addictions, social work, psychology or human services related discipline; plus two years experience in a Counselor II level position; certification; experience working with adult and/or adolescent population preferred.

The following credentials may substitute:

  • Bachelor or Arts/Science Degree in human service related discipline; plus three years experience in a Counselor II level position; certification.
  • Associate of Arts/Science Degree or certificate in addictions; plus five years experience in Counselor II level position; certification.
  • Other relevant credentials may be accepted at the discretion of the Program Director/Facility Director and Human Resources Manager.
